Technology Equipment Removal Agreement
Please complete this form for each child that will be checking out a device.
By typing in your name below (must be a parent or guardian), you agree to the following terms:
Do not mark the device in any way with markers, stickers, etc. Each device is labeled with a district identification barcode or serial number. Do not remove this sticker or number.
Devices should always be used on a table or desk. They are not to be used on a bed or on someone’s lap.
Do not insert foreign objects (paperclips, pens, etc.) into the ports (openings) of the device.
Do not eat or drink near the device. There should never be any food or drink around the device.
Store your device computer when not in use. Excessive scratches and scuffs to the device’s exterior can result in repair fees assessed to the student.
Make sure hands are clean before using the device – the Chromebook’s glass screen should be regularly wiped clean with a dry clean soft cloth – microfiber cloth is recommended but any soft cotton fabric wil work. Please do NOT use commercial liquid or spray cleaners on the iPad screen.
When charging cable needs to be connected, be sure to line it up correctly when inserting and removing. Students are responsible for damage to the charger port or connector pin resulting from mishandling.
Absolutely no other apps should be installed on these devices. The only settings that should be adjusted are the device’s wifi connection.
The parents accepts the full replacement cost of the Chromebook should it be damaged, lost or stolen.
If you have problems with your device, stop using the device and ask Chris Mara or a teacher for help.
